The conditions most likely to shorten your life announce themselves late. High blood pressure causes no symptoms while it damages your kidneys and heart. Type 2 diabetes can develop over years unnoticed. High cholesterol is silent until the day it is not. An annual physical is how those problems get caught while they are still easy to reverse. A useful physical does several things at once. It establishes a baseline, which matters more than any single reading — a blood pressure of 138 means something different in someone who has always run 115 than in someone consistently at 140. It catches drift: small annual changes in weight, kidney function, or blood sugar that no one would notice in isolation but that form a clear trend across three years of records. It keeps preventive care on schedule, since colonoscopies, mammograms, and vaccines are easy to postpone indefinitely without someone tracking them. And it creates a low-stakes opportunity to raise the things you would never book a visit for — fatigue, sleep, mood, drinking, sexual health. Those conversations frequently turn out to be the most valuable part of the hour. Perhaps most importantly, it means that when something does go wrong, you already have a physician who knows your history rather than starting from a blank chart.
