Integra Primary Care’s website contains no contact forms, no patient intake forms, and no symptom questionnaires. We do not collect, store, or transmit personal health information through this website. All clinical communication, scheduling, and record access occur through separate HIPAA-compliant systems described below.
We use Google Analytics to understand general website usage — which pages are visited and how visitors arrive. This collects technical data such as browser type, approximate region, and pages viewed. It is not linked to any patient record, and we do not use advertising or social media tracking pixels on this site. You may opt out using Google’s browser add-on or your browser’s privacy settings.
As a healthcare provider, Integra Primary Care maintains a full Notice of Privacy Practices describing how medical information about you may be used and disclosed, and how you can access that information. You have the right to access your medical records, request corrections, request restrictions on certain uses and disclosures, receive an accounting of disclosures, and file a complaint if you believe your privacy rights have been violated. Complaints may be made to our office or to the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
